PROGRAM DESCRIPTION
Once a patient with acute decompensated heart failure (HF) has been stabilized in the emergency department (ED), what are appropriate next steps to ensure optimal outcomes without overburdening the healthcare system? Although many HF patients can be safely discharged to home care, others are at markedly increased risk for readmission and even death. In part 2 of this 2-part webcast, 3 experts—an ED physician, a hospitalist, and a cardiologist—discuss approaches to management of acutely worsening HF, with a focus on strategies for risk assessment and evidence-based, guidelines-directed recommendations for designing a care plan that will promote the best possible patient outcomes.
